To begin
the application process for the Therapeutic Riding Program please
download the files, complete and then return back to the stables. The
Physician's Form needs to be filled out by the rider's physician, signed
and dated. The Health History and Waiver/Release Forms need to be completed by the parent/guardian or adult rider.
Please check and make sure both sides of the forms are filled out and
signed.
